MANAGING RISK FOR ON-LINE WORK Copy To: John Anderson "In order to conservatively model risk following the identification of a possible common mode failure in the Auxiliary Feedwater Recirc line, the PRA group has entered risk factors to model this possible failure."
In the interim, it is prudent for us to establish/reinforce guidance for scheduling on-line work so that overall risk is managed acceptably. The following expectations, to the extent practical, will help manage further risk accumulation for 2002.
?? Care shall be taken in scheduling out-of-service time for risk-significant equipment in order to minimize simultaneous unavailability of such equipment.
?? Tagging activities shall be arranged to minimize out-of-service time for risk-significant equipment; i.e., Operations should tag equipment just prior to start of work, and clear tags to allow prompt return-to-service following completion of work.
9? Maintenance and job support coverage shall be arranged in order to minimize out-of-service time for risk-significant equipment. MTAM 2.1 guidance for coverage for emergent work should be considered. Actual plant configuration should be considered when setting the

?? Emergent risk-significant equipment failures shall be promptly evaluated with Safety Monitor to allow early identification of risk mitigating measures.. Operations should perform this evaluation, and request support from NSA to identify effective mitigation measures.
