Dear Ms. Landrum:
This letter is provided as a follow-up to the voicemail originally left for Ms. Shannon Kennedy 26 February 2003, 09:15 hours, and the follow-up notification conversation with Ms. Jan Malone, 27 February 2003 concerning the release of fire protection water from the above referenced facility to Squaw Creek Reservoir. The following information is provided concerning this release:

02/25/2003, 17:05 - 19:02 hours. This is the automatically logged run time of a fire water pump in response to the loss of water from the system; A single sprinkler head malfunctioned and was activated in an unoccupied on-site storage building, flooding a portion of the building, and draining off to Squaw Creek Reservoir;

Estimated 8,000 to 9,000 gallons may have reached Squaw Creek Reservoir. Firewater tank drawdown indicated a loss of 11,500 gallons. Estimate is based upon this value minus an estimate of water contained in the building. The water in the fire protection system is normally maintained with an approximate concentration of the biocide, Bulab 6002 (CAS #31075-24-8), of 50 mg/L as product.
This biocide does not contain any components or substances listed by SARA 302, SARA 313, CERCLA, RCRA, or the CWA. There were no dead or stressed fish identified in subsequent inspections of the reservoir in the vicinity of the release.
The preliminary cause determination is that the sprinkler head may have inadvertently activated due to freezing.
Immediate response was to isolate the fire protection water to this building, and repair leak.
A root cause analysis is currently in progress.
