A. Technical Issue:
Point Beach identified a potential common mode failure associated with the AFW system in November, 2001. The specific failure mechanism was associated with inadequate operator actions in response to a loss of instrument air. Loss on non safety-related instrument air could also have caused loss of AFW and primary bleed and feed capability.
B. Licensee Identification:
Licensee completed formal notification process for the potential common mode failure November 29, 2001. Initial discovery phase started in July, 2001.
C. NRC Response and Inspection Activities:
NRC Region III develops charter for Special Inspection on November 30, 2002.
Special Inspection occurs from December 3, 2001 through February 28, 2002. Report conclusion documented preliminary conclusion of a RED finding.
The report documents two apparent violations of 10 CFR Part 50, Appendix B, Criteria V and XVI.
A Regulatory Conference is conducted on April 29,2002. The Point Beach licensee agrees with the risk characterization of the issue as RED, but requests that the NRC consider the issue to be an Old Design Issue.
On July 12, 2002, NRC issues a Final Significance Determination letter, classifying the Issue as having RED safety significance. NRC informs Point Beach of intent to perform additional inspection to determine whether the issue should be treated as an Old Design Issue.

- 2. Current Special Inspection (Potential Common Mode Failure of AFW Pumps Due to Recirculation Line Orifice Plugging)
A. Technical Issue:
On October 24, 2002, Point Beach personnel conducted a surveillance run of the "A" motor driven AFW pump. Flow was less than the acceptance criteria specified in the surveillance test, but above operability limits. On October 29, 2002 Point Beach, following engineering investigation into the 10/24/02 AFW surveillance results, notified the NRC of a potential common mode failure of the AFW system associated with the potential for plugging of the AFW recirculation line flow orifices.
On October 31, 2002, NRC Region III initiates a Special Inspection to evaluate issues associated with the Point Beach AFW system.
B. NRC Special Inspection Activities:

- 1) WHITE Emergency Preparedness - NRC identified an inadequate EP exercise critique associated with a Risk Significant Planning Standard on February 12, 2002. A Regulatory Conference was scheduled for June 25, 2002; however, on June 24, 2002, Point Beach informed NRC that additional information would be submitted to the NRC in writing, in lieu of conducting a Regulatory Conference. On September 12, 2002, the NRC, after consideration of the written response from Point Beach, issued a Final Significance Determination letter classifying the EP critique issue as a WHITE finding.
- 2) RED Mitigating Systems (see description above for RED finding associated with the potential common mode failure of the AFW system).
C. Unit 2 Inspection Findings:
- 1) WHITE Emergency Preparedness: same issue as Unit 1
- 2) RED Mitigating Systems: same issue as Unit 1
- 3) WHITE Safety Injection (SI) Pump Failure: On May 2, 2002, SERP held to evaluate Sl pump gas binding which resulted in an SI pump failure. Gas buildup was due to leaking check valves between the pump and the SI accumulator. The Final Significance Determination letter was issued on June 13,2002, documenting the issue as having WHITE significance.
