Cook Nuclear Plant Units 1 and 2 NON-PCB TRANSFORMER OIL SPILL NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM AND GROUNDWATER DISCHARGE AUTHORIZATION NOTIFICATION Indiana Michigan Power Company is the holder of National Pollutant Discharge Elimination System (NPDES) permit M10005827 and Groundwater Discharge Authorization (GDA) M00988 for the Donald C. Cook Nuclear Plant (CNP).
Section 3.2 of Part II of Appendix B (Environmental Technical Specifications) to Facility Operating Licenses DPR-58 and DPR-74 requires violations of the NPDES Permit to be reported to the Nuclear Regulatory Commission (NRC) by submittal of copies of the reports required by the NPDES Permit.
In a letter dated February 3, 2003, the Michigan Department of Environmental Quality (MDEQ) was notified of a spill of approximately 20 gallons of Univolt 60 mineral non-PCB transformer oil. The oil spill occurred during work activities associated with the replacement of the Unit 1 main transformer.
Notification was provided to the MDEQ per Part II, Section C.7 of the NPDES Permit and Part II, Section C.7 of the GDA. In accordance with the Environmental Technical Specifications, a copy of the report is being provided to the NRC as an attachment to this letter.
This letter contains no new commitments.

This notification is made pursuant to Cook Nuclear Groundwater Discharge Authorization M 00988, Part II. Section A. 7, Spill Notification.
On January 24, 2003 at 1500 hrs, an estimated 20 gallons of non PCB transformer oil was released to the ground during transformer replacement. Workers were removing an instrument from the spare Reserve Feed Auxiliary Transformer (RATS) when approximately 20 gallons of Univolt 60 mineral oil spilled to a containment berm and to the ground adjacent to the containment berm. An isolation valve for the instrument was thought to be closed when in fact it was not. When the workers unscrewed the instrument from its location, it popped off into the hands of one of the workers. The oil sprayed out the valve with some of it landing inside the containment and some spraying beyond it to the frozen ground. The valve was closed, stopping the oil release. Plant personnel were dispatched to contain and clean up the spill. The oil contaminated sand and gravel was collected and will be properly disposed. This spill does not pose a threat to the environment, public health or safety. Work crews have been briefed to employ self-checking behaviors when working on oil containing equipment.
