This report is submitted pursuant to 10 CFR 26.71(d).
Attachment 1 includes summaries of management sanctions imposed, actions completed to improve the program, events reported to the NRC, and our random testing rate. Attachment 2 contains the standard FFD data forms for North Anna Power Station, Surry Power Station, Innsbrook Technical Center (corporate),
Westinghouse Electric Company and Institute of Nuclear Power Operations (INPO).
