ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a White Significance Determination Process (SDP) finding and associated Notice of Violation will be issued on or around March 4, 2003, to FirstEnergy Nuclear Operating Company as a result of inspections at the Perry Nuclear Power Plant. This WHITE finding, an issue with low to moderate increased importance to safety, which may require additional NRC inspections, involved the failure of the high pressure core spray system to start during routine surveillance testing on October 23, 2002. The licensees failure to properly align cell switch contacts during initial installation of the pump breaker in 1994 and during subsequent inspections is identified as a violation of Technical Specification 5.4 for failure to follow maintenance procedures. Accordingly, a Notice of Violation associated with the WHITE finding is being issued.
It should be noted that the licensee has not been specifically informed of this action. The schedule of issuance and notification are:
