REPRINT OF NRC FORM 396 Enclosed are several copies of NRC Form 396, Certification of Medical Examination by Facility Licensee, which was reprinted in January 2003. Additional copies may be obtained by calling Beverly Martin in the Office of the Chief Information Officer, at 301-415-5877, or, if you prefer, by electronic mailing her at Forms@NRC.gov or BAM1@NRC.gov.
This form has been reprinted to change the OMB expiration date (top right corner) to 01/31/2006.
NRC Form 396, and NRC Form 398, Personal Qualification Statement-Licensee, are available in *.pdf format from the NRCs Operator Licensing web site at http://www.nrc.gov/reactors/operator-licensing/licensing process.html. Although the completed forms cannot be submitted to the NRC electronically, they can be filled out electronically using the Adobe Acrobat Text Annotation Tool. Note that this tool is not included in the Adobe Reader.
