SUBJECT:

SENIOR MANAGEMENTS REVIEW OF THE DAVIS-BESSE LESSONS LEARNED TASK FORCE REPORT The purpose of this paper is to inform the Commission of the actions being developed and implemented as a result of senior managements review of the Davis-Besse Lessons Learned Task Force Report recommendations.

BACKGROUND:

A Senior Management Review Team (RT) was established on October 3, 2002, to review and evaluate the Final Lessons-Learned Task Force (LLTF) Report on the Degradation of the Davis-Besse Nuclear Power Station Reactor Pressure Vessel Head. The RT, led by Carl Paperiello, Deputy Executive Director for Materials, Research, and State Programs, provided the results of their review in a memorandum to me, dated November 26, 2002. The report is attached. The RT agreed with the LLTF recommendations, with two exceptions, and prioritized the recommendations according to the significance of their contribution to the Davis-Besse vessel head degradation. The RT also organized the recommendations under four overarching categories. Action plans and/or focused activities are proposed for each of the categories to address the recommendations.

DISCUSSION:

I believe the actions recommended by the RT will address the LLTF report recommendations and have tasked the staff to develop action plans in accordance with the RT proposal. The proposed implementation time frames for the high priority activities are provided in the attachment and completion schedules will be provided to you when available. A semi-annual status report to assess the progress of activities will be initiated which will include a determination of whether revisions are needed. The remaining activities will be integrated into the operational planning activities for the lead offices using the agencys Planning, Budgeting, and Performance Management (PBPM) process.

The four overarching categories for action are: (1) Assessment of Stress Corrosion Cracking, (2) Assessment of Operating Experience, Integration of Operating Experience into Training,

2 and Review of Program Effectiveness, (3) Evaluation of Inspection, Assessment, and Project Management Guidance, and (4) Assessment of Barrier Integrity Requirements.

In a January 3, 2003, memorandum, I tasked the Office of Nuclear Regulatory Research (RES) and the Office of Nuclear Reactor Regulation (NRR) to jointly develop an overall plan to implement the recommended course of action provided in the November 25, 2002, RT memorandum. This plan is due to me by February 28, 2003, and will clearly delineate work schedules and responsible offices for each of the for overarching categories. The impact on existing work and resource implications will also be included.

Several of the LLTF recommendations for action are already underway or have been completed by the program offices. The program offices will provide a status of the actions completed to satisfy LLTF recommendations in the semi-annual status reports.
