Cook Nuclear Plant. Spills are required to be reported to the Nuclear Regulatory Commission (NRC) by Appendix B (Environmental Technical Specifications) to Facility Operating Licenses DPR-58 and DPR-74, Part II, Section 3.2.
In a letter dated November 26, 2002, the Michigan Department of Environmental Quality (MDEQ) was notified that a leak of approximately two gallons per minute that was about 12 percent sodium hypochlorite by volume discharged into the forebay for approximately nine hours.
These notifications were provided to the MDEQ per Part II, Section C.7 of the NPDES Permit and Part II, Section A.7 of the GDA. Accordingly, a copy of this report is provided to the NRC as the attachment to this letter.

7, Spill Notification.
On November 16, 2002 at approximately 00:15, a one half inch diameter Teflon tubing supply line to the service water chlorination pump failed during continuous chlorination operations. A leak of approximately two gallons per minute of
-12% sodium hypochlorite discharged into the forebay, where it was diluted and carried through the circulating water system and finally discharged into Lake Michigan through Outfall 001 for approximately 9 hours1.041667e-4 days <br />0.0025 hours <br />1.488095e-5 weeks <br />3.4245e-6 months <br />. The discharge flow from the Unit One circulating water system (Outfall 001) is approximately 690,000 gallons per minute.
The Total Residual Oxidant (TRO) concentration at Outfall 001 ranged from 0.282 mg/l to 0.386 mg/l during the 9 hour1.041667e-4 days <br />0.0025 hours <br />1.488095e-5 weeks <br />3.4245e-6 months <br /> period. Since TRO is measured at the beginning of the discharge tunnel, we expect a significant reduction in the TRO at the end pipe due to biofouling on the walls of the discharge pipe.
The discharge pipe is more than 1000 feet long.
Approximately 1080 gallons of the -12% sodium hypochlorite solution was discharged.
The leak occurred from approximately 00:15 to 09:15 on November 16, 2002.
Plant personnel discovered the leak on a routine chlorine pump room tour at approximately 09:00 and responded immediately to secure the sodium hypochlorite tank. The discharge exceeded the NPDES continuous permit limit of 0.038 mg/l TRO.
This spill did not pose a human health threat as the concentration of sodium hypochlorite was less than normal drinking water levels. The high velocity discharge, the large dilution from the circulating water system, the sandy bottom and open lake provided adequate dilution and absorbance to prevent any immediate or long-term environmental damage.
Notifications were made to the National Response Center on November 16, 2002 at 11:50 and the Pollution Emergency Alert System (PEAS) at 12:05.
A follow-up call was placed to the Kalamazoo District Office on November 18, 2002.

MI 0005827 We have taken actions to repair the supply tubing and are investigating preventative actions that could minimize future failures of this tubing.
All sodium hypochlorite tubing is being checked to ensure adequate integrity.
Continuous chlorination has been suspended until an alarm is energized on the continuous total residual oxidant monitor.
