ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a WHITE Significance Determination Process (SDP) finding and associated Notice of Violation will be issued on or around July 23, 2002, to Exelon Generation Company, LLC, as a result of inspections at the Braidwood Station. This WHITE finding, an issue with low to moderate increased importance to safety, which may require additional NRC inspections, involved the licensees failure to take adequate corrective actions for Unit 1 pressurizer power operated relief valve (PORV) air accumulator check valve failures.
The failure of the licensee to promptly identify and correct this significant condition adverse to quality was identified as a violation of 10CFR50, Appendix B, Criterion XVI, Corrective Action.
This resulted in several extended periods where the unit was operated in a condition where the pressurizer PORVs may not have been able to perform their intended safety function of opening following events which resulted in isolation of instrument air to the containment or loss of the service air compressors.
In accordance with the NRC Action Matrix, overall plant performance for Braidwood Unit 1 has been determined to be in the Degraded Cornerstone column due to this White finding and a previous White performance indicator for the Safety System Unavailability, Heat Removal System (Auxiliary Feedwater).
It should be noted that the licensee has not been specifically informed of this action. The schedule of issuance and notification are:
