Amended Fitness for Duty Performance Data Report for the Period (1/01/01-6/30/01)

Dear Sir or Madam:

For the reporting period of 1/01/01 - 6/30/01 data entry input errors, resulting in incorrect reporting of actual For-Cause: Post-Accident Testing Results, were self-identified by annual Departmental self-assessment of the Fitness for Duty Program. Specifically, the number of For-Cause: Post Accident testing results reported for Long-Term Contractors (1), and Short-Term Contractors (2), was incorrect.

A detailed review of data supporting the subject reporting period confirmed no post accident for-cause testing was performed, and the data should have been reported in the Pre-Access / # Tested fields.

The required data corrections have been completed and are reflected in the enclosed amended Performance Data Report for the subject period. I may be reached at (802)258-5838 for questions regarding the amended report, if any.
