CAUSE/CORRECTIVE ACTION/COMMENTS:
EVENT 2002-01 CAUSE:
Scheduled Refueling Outage CORRECTIVE ACTION:
Cooldown to 110 Deg. F, vessel disassembly, fuel off load, mid-loop maintenance, re-load core, reassemble vessel, fill and vent reactor vessel, and complete prerequisites for mode changes.
CRITICAL PATH WAS:
Vessel disassembly, fuel off load, mid-loop work, fuel load and vessel reassembly, Turbine Rotor replacement, and Main Generator rewind.
EVENT 2002-02 CAUSE:
Forced Outage due to high vibration on Main Turbine bearing #7. Vibration was induced through the opening of steam dump valve 1-PV -507C. There was a subsequent Reactor trip due to operational problems with Steam Generator Water Level Control.
CORRECTIVE ACTION:
Steam Dump repair, return unit to service. Reference LER 1-2002-03 for actions associated with the reactor trip.
CRITICAL PATH WAS:
Stabilize plant, isolate steam dump, reactor trip recovery, unit startup.
