Exam Analysis Questions missed by > 50% of the candidates

  1. 7 Training focused on Red and Orange path criteria. Candidates did not know the criteria separating Green and Yellow paths. Remediated during exam review.
  1. 10 Candidates were either not sure of the function of the rod stop bypass switch, or believed the loss at VIAC would fail the NIS channel low without bringing in the overpower rod stop bistable. Remediated during exam review.
  1. 27 Question was based on a note within the SBO diesel normal operating procedure. Candidates did not recollect the note. Remediated during exam review.
  1. 29 Question tested a note within the new tube leak procedure. Remediated during exam review.
  1. 30 Question tested the physical relationship between the Turbine Driven Main Feed Pump EAL speed controller and the manual speed controller.

This was a difficult question. Remediated during exam review.

  1. 44 Question tested a minor step in E-1, which contained a different remediation strategy than the CTMT high radiation EOP. Remediated during exam review.
  1. 45 Candidates were unsure of required action, and unsure if computer displays thermal power on loss of calorimetric input. Remediated during exam review.
  1. 50 Candidates did not realize that suction source was inadequate for motor driven man feed pump to prevent the trip. Remediated during exam review.
  1. 83 Candidates were not sure if 2200°F marked the point of zirc water reaction accelerating, or becoming auto-catalytic. Remediated during exam review.
  1. 97 Candidates were aware of the good practice of checking status trees at procedure transitions and/or weren't aware of the requirement to check status trees during significant changes in plant status. Remediated during exam review.

SRO #56 Candidates did not recollect the minimum pressure in E-0 that allows stopping the second charging pump. Remediated during exam review.

SRO #58 Candidates were aware of the normal practice of not discharging when the tide is coming in, and were not aware of the required recirc time for a waste test tank. Remediated during exam review.

SRO #59 Candidates interpreted the steady RCS pressure as being indicative of the leak being isolated. Remediated during exam review.

SRO #69 Candidates did not know the SBO diesel is not a credited power source for the site Area Emergency classification. Remediated during exam review.

NOTE The post exam critique was attended by both the candidates and the LOIT Coordinator, to identify where the program may not have adequately prepared the students.
