Fitness for Duty Program for the Period 1/01/01 - 6/30/01 For the reporting period of 1/01/01-6/30/01 errors were discovered in the reporting software used to compile the above subject report. Specifically, the number of random tests reported for the period was found to be incorrect. Event Report Number 2001-2016 was initiated and the Information Technology Department made corrections to the software. A follow-up Surveillance was also conducted by Vermont Yankee QA Department to ensure the validity of the random generator. As a result, this addendum is submitted to reflect the corrections for the above identified reporting period.
Please find enclosed an amended Semi-Annual Performance Data Report for the subject period.
