For three consecutive exercises (6/94, 6/98, and 9/99), repair teams had been dispatched with a radiological release in progress without their knowing of the release.

Problem Identification and Resolution a
The 1995 and 1996 QA audits reports of the EP program were nearly identical Following the 6198 exercise, there was no debrief with players and the critique of the OSC/TSC were not self-critical to result in improvements to those areas The 9/22/99 exercise critique did not identify many of the NRC findings in the OSC/TSC Because of a repeat weakness during the 9122199 exercise a violation was issued.

Conclusion:==
Prior to issuing a license, the NRC must have reasonable assurance that the licensee's emergency plan is adequate and can be implemented. There are 16 planning standards listed in 10CFR50.47(b). IOCFR50.47(c)(1) states that failure to meet the applicable planning standards may result in the Commission declining to issue an operating license. A review of EP at IP2 for the past two years reveals problems that significantly challenge whether six of those planning standards have been met or implemented up to and including failing to meet or implement six of those planning standards (ERO augmentation, emergency classifications, public information, onsite protective measures, exercises, and training).
