Re: SAI-M90 we understand that, to supplement the application, a cover letter to come will contain a statement that the previous commitments for the product and its distribution have not changed with the exception of itemized changes. Below it a quick version of the kind of thing we are looking for. I tried to list out the main points as illustrations.
There have been no changes to the product or its distribution since last registration (including device design, quality assurance/control, distribution commitments), with the following exceptions:
- 1. company name change from XXX to YYY (Eli, here is where you might need to explain how it is that they never were Palomar. If there have been ownership changes involved, rather than just change the name, then need also to address items in NRC's Information Notice IN 89-25, Rev. 1)
- 2. the registered product will be the (whole device) instead of only the module. additional drawings are submitted to describe the (outside the module part of the device)
- 3. the tamperproof screws changed (did we change these?, I can not remember for certain)
- 4. labels were changed to.... (since the whole device is the registered product now, will need to revisit labeling to ensure in compliance with 10 CFR 32.29(b)
