Francisville, West Feliciana Parish.
Dear Mr. Spell:
Final LPDES permit LA0042731 for Entergy Operations, Inc.'s River Bend Station was effective on October 1, 2011. However, the permit contained errors in the monitoring and reporting requirements language for several of the pollutant parameters at Outfalls 201 and 003. The following modifications have been made to correct the errors:

Outfall 201 - Two new footnotes have been added to clarify the proper monitoring frequency and Discharge Monitoring Report (DMR) reporting requirements for TSS, as it relates to maintenance activities. If maintenance activity discharges occur during the quarter, quarterly total suspended solids (TSS) sampling is not required. TSS must be sampled weekly and the results reported on the 201A DMR. A notation shall be made on the 201Q DMR stating the dates maintenance activities occurred and on which month's DMR TSS results were reported.
Changes were made to Part I, Pages 5 and 6 of 17 of the permit. See footnotes 5 and 6.

Outfall 003 - Two new footnotes have been added to clarify the proper monitoring frequency and DMR reporting requirements for Flow, Oil & Grease, and pH, as related to low volume wastewater discharges. If low volume wastewater discharges occur during the quarter, quarterly monitoring for Flow, Oil & Grease, and pH is not required. Flow, Oil & Grease, and pH must be monitored monthly and the results reported on the 003A DMR. A notation shall be made on the 003Q DMR stating the dates low volume wastewater discharges occurred and on which month's DMR Flow, Oil & Grease, and pH results were reported. Changes were made to Part 1, Pages 12 and 12A of 17 of the permit. Please note that the footnotes for Outfall 003 were renumbered to accommodate these changes. See footnotes 2 and 3 for changes to the monitoring and reporting requirements.

Please replace the appropriate pages in LPDES permit LA0042731 with these revisions. The permit modification fee has been waived because this is an Agency initiated minor modification of LPDES permit LA004273 1.
Monitoring results should be reported on a Discharge Monitoring Report (DMR) form per the schedule specified. A copy of the form to be used is attached for your convenience.

During the period beginning the effective date of the modification and lasting through the expiration date the permittee is authorized to discharge from:
Internal Outfall 201, the intermittent discharge of treated sanitary wastewater; also, during maintenance activities, sanitary wastewater may be combined with previously monitored hydrostatic test wastewater, wastewater from floor drains of the control building and the diesel generator oil/water separator (and other low volume wastewaters as defined in 40 CFR 423), and maintenance wastewaters and may be routed to Outfall 002 (*4).
Such discharges shall be limited and monitored by the permittee as specified below:

Future water quality studies may indicate potential toxicity from the presence of residual chlorine in the treatment facility's effluent. Therefore, the permittee is hereby advised that a future Total Residual Chlorine Limit may be required if chlorine is used as a method of disinfection. In many cases, this becomes a NO MEASURABLE Total Residual Chlorine Limit.

Samples shall be representative of discharges occurring during maintenance activities.

Discharge Monitoring Reports must be submitted for BOTH Outfall 201 operational scenarios.
If either scenario does not discharge within the monitoring period, mark "no discharge" on the, top right hand corner of the DMR for that operational scenario.

If no maintenance activity discharges occur during the quarter, total suspended solids (TSS) must be sampled quarterly and the results shall be reported on the 201Q DMR.
A notation shall be made on the 201A DMR stating that no maintenance activities occurred during the quarter.

Outfall 003, the intermittent discharge of stormwater runoff from the reactor building, turbine building, services building, clarifiers, main transformer yard and auxiliary transformer yard; maintenance wastewaters including but not limited to flushing of piping systems and vessels (including Fire Protection Water Supply System and Automatic Sprinkler System); low volume wastewaters including but not limited to reverse osmosis reject water from the standby service water polishing system, effluent from floor drains within power plant buildings (domestic potable water, well water, reject mobile reverse osmosis and fire suppression'water treated in the fire pump house oil/water separator), air compressor condensate, and reverse osmosis reject water from makeup water polishing system; air conditioning condensate, previously monitored hydrostatic test wastewater, and de minimis quantities of cooling tower drift/mist (*6).
Such discharges shall be limited and monitored by the permittee as specified below:

There shall be no discharge of floating solids or visible foam in other than trace amounts, nor of free oil or other oil materials, nor of toxic materials in quantities such as to cause acute toxicity to aquatic organisms. Furthermore, there shall be no visible sheen or stains attributable to this discharge.
Samples taken in compliance with the monitoring requirements specified above shall be taken at the following location(s):
Outfall 003, at the point of discharge from the plant drainage ditch system along the East Creek prior to combining with other waters.

(1) discharges which do not include low volume wastewaters; and (2) discharges which include low volume wastewaters. If either scenario does not discharge within the monitoring period, mark "no discharge" on the top right hand corner of the DMR for that operational scenario.
