Dear Mr. Meyer:
In accordance with the requirements of 10CFR55.57, Renewal of Licenses, Section (a)(4),
and 10CFR50.59, Licensed Operator Renewals were submitted during the period 1994 through 1996. During a subsequent review of licensed operator requalification tra:ning records, it was determined that individuals had missed some required training topics.
Review of the topics missed for the 1994 through 1996 period was performed. The Operations and Training Managers evaluated the topics, reviewed other training performed, and determined that operator qualification was not in quertion.

The instances of missed training had not been identified earlier due to reliance on results from pre-established computer database queries tnat were later freund to be inadequate. These j
queries would only indicate an absence from required trainirg if the code ' ABS' was f
l entered into the database. Training department personnel erroneously thought that any
/
entry other than ' PRES' would indicate an absence and consequently did not always enter the ' ABS' code. The inadequacies of these queries were recently discovered during a
/
follow up to a OA inspection and subsequent implementation of a new computer network M database application / program (2/28/97).
The new computer network database application accurately identified the missed training.
A characterization of the missed topics and numbers of missed topics are provided in Attechment 1. The corrcatM actions that will prevent recurrence of this type of event are provided as Attachment 2.

ATTACHMENT 1 There are currently a total of 54 Licensed Operators at JAF.
Thirty-one are assigned to operating shifts and the remaining 23 i
are staff licenses, assigned to duties that are generally conducted outside of the control room.
The distribution of the shift and staff licenses is as follows:

21 During the period from 1994 through 1996, eighteen (6) License renewals were submitted of which ten (10) were staff licenses that remained inactive throughout the entire period.
Analysis of Licensed Operator requalification training for the period from 1992 through 1996 indicates that the queries used to poll the Training Database did not accurately ' indicate missed attendance.
The queries only indicated an absence if it was specifically documented as ' ABS'.
The training department staff was.not sensitive to this and thought that entering ' ABS',
'N/A' or no entry were all appropriate means of documenting absences.
This resulted in entries of
'N/A' or non-entries not being identified as absences.
The new Training Records Database was placed into service in February, 1997.
The queries developed for this database correctly identified all absences.
Using the new queries, a review of the training records database for the period from 1992 through 1996 indicated numerous instances of absence.
For 1992 and 1993, subsequent follow-up review of hard copy-files identified that there were no unresolved absences.
For 1994 through 1996, similar follow-up of hard copy files resolved the missed topics with the exception of those identified below.
A detailed review of the topics missed for the last rer]ualification period (1995 and 1996) indicates that there is not a concern. with Licensed Operator qualification. During that two-year period, 9 of the 54 Licensed Operators had missed only 1 training topic (exclusive of
- quizzes, practice examinations and simulator evaluationo); 2 of 54 had missed 2 training topics; and 8 of 54 were delinquent on procedure reviews.
These are not considered to represent serious deficiencies in operator qualification because of the relatively low numbers of topics missed. Also during 1995 and 1996, all licensed individuals participated in tha annual exam process.
If knowledge weaknesses were identified, t~
individuals were remediated or placed in an accelerated requcin ication training program, as applicable.
The missed procedure reviews are not considered significant from the perspective that Licensed Operators at JAF review emergency and abnormal operating procedures annually versus biennially and also
-because changes to all procedures are reviewed separately via the Operations Department Night orders.
