Restxmse to Notice of Violation Violation A.
Technical Specification 6.8.A states, in part, that procedures shall be I
established, implemented, and maintained for the Fire Protection Program.
Procedure WACP-10.1.10, " Control of Combustible and Finmmnhle Materials," Sections 6.3.7 and 8.2, which, in part, implements this TS requirement, states that: (1) Fire Protection Supervisor, " Establish Alert and Action levels for the Storage Areas..."; and (2) "The Fire Pmtection Supervisor, with assistance from the Fire Protection system Engineer, shall evaluate the quantity of combustible and flammable materials of the storage areas... The evaluation should compare the quantity of materials to the alert and action levels established for the area."
Contrary to the above, as of September 8,1993, WACP-10.1.10 was not adequately implemented in that: (1) Fire Protection Supervisor failed to establish Alert and Action levels for the Storage Areas; and (2) the Fire Protection Supervisor had not evaluated the quantity of combustible and flammable materials of the storage areas, comparing quantities of materials present in each area to alert and action levels established for each area.

i Admission or Denial of the Alleged Violation The Authority agrees with this violation.
The Reason for the Violation Personnel error is the primary reason for both failure to establish Alert and Action levels for temporary storage of combustibles in interim storage areas and the failure to perform transient fire loading calculations for materials in the interim storage location.
i Procedure changes had been initiated in order to improve the control of transient combustibles stored within the power block in interim storage locations. The changes were implemented, however, fire protection personnel failed to follow procedure.

Corrective Steos Taken to Avoid Further Violation Alert and action levels for storage of transient combustibles were established for the designated interim storage areas within the power block on September 9,1993. A fire inspector's night order book cyry provided all fire inspectors appropriate guidance and direction for implememation of the Alert and Action levels. Input to the Fire Inspector Training program was provided to cover guidance for tours of the Turbine Track Bay, Interim Waste Storage Area.
The calculations were performed for all incomplete interim waste storage logs and reviewed by the Fire Protection Supervisor. These were completed on September 11, 1993.
An Administrative Procedure, AP-02.04, " Control of Procedures" was implemented in June,1993. One purpose for this procedure was to ensure l
adequate implementation plans for both new procedures and applicable

procedure revisions. Responsible procedure owners were designated for all procedures. Had this administrative guidance been in place at the time the Control of Combustible and Flammable Materials procedure was revised, the t
probability of inadequate procedure implementation would have been significantly reduced.
l A review of the process fc,r controlling the quantity of combustibles in interim storage areas identified improvements to three procedures which would facilitate more timely identification and disposition of transient combustibles.
These improvements will be implemented by November 19,1993.
Additionally, an Interim storage area for combustible and flammable materials outside the power block has been designated. This facilitates timely and efficient removal of unnecessary trantient fire hazards from the power block.
