NOTICE OF VIOLATION Docket No. 55-6236 License No. SOP-4301-4 IA 96-065 During an NRC inspection conducted on September 14 through October 4. 199 and an NRC Office of Investigations investigation completed on June 28. 199 a violation of NRC requirements was identifie In accordance with the

"General Statement of Policy and Procedures for NRC Enforcement Actions."

NUREG-1600. the violation is listed below:

Senior Reactor Operator's License. SOP-4301-4. Docket No. 55-6236 (expired on December 28. 1995) required that you comply with all applicable rules. regulations. and orders of the Commissio Surry Nuclear Power Station Technical Specification 6.4 required that detailed written procedures be provided for activities which would have an effect on nuclear safety and that those procedures be followe Procedure l-OP-RC-011. -Pressurizer Relief Tank Operations. Revision Section 5.5. established the methods for venting the Pressurizer Relief Tank (PRT).

Steps 5.5.5 and 5.5.6 required that a polyethylene hose be connected from Valve No. l-RC-ICV-5025 to the nearest containment purge exhaust duct and Valve No. l-RC-HCV-1549. PRT Vent. be close respectivel Contrary to the above. on September 13. 1995. Mr. Robert C. Allen deliberately failed to comply with the provisions of his senior reactor operator license issued by the NR Specifically. Procedure 1-0P-RC-011, Pressurizer Relief Tank Operations, Rev. 1, Section 5.5, establishes the method for venting the pressurizer relief tank to the Vent System. Steps 5.5.4, 5.5.5, and 5.5.6, require that a Gaseous Group Release Permit be obt_ained for venting the pressurizer relief tank to the Vent System; a poly hose be connected from valve 1-

Mr. Allen deliberately violated established operating procedures by initiating a release of the Unit 1 pressurizer relief tank without a permit. Approved detailed written procedures were not followed, or properly modified, to perform venting of the Unit 1 pressurizer relief tank as evidenced by the followi_ng: No Gaseous Group Release Permit was obtained for venting the pressurizer relief tank to the Vent Syste.

A poly hose was not connected from valve 1-RC-ICV-5025 to the nearest containment purge exhaust duc.

Valve 1-RC-HCV-1549, PRT Vent, was not closed.
