Review of QA For Extended Construction Delay The WNP-1 Preliminary Safety Analysis Report (PSAR), Chapter 17 specifies that WNP-1 shall implement an overall Quality' Assurance Program (QA Program) for the design, procurement, construction and operation of Supply System's Nuclear Project No. 1 (WNP-1) in accordance with the requirements of Appendix "B" of 10 CFR 5 The licensee's extended construction delay program had been previously reviewed and inspected in 1984 and 1985 by NRC RV. The inspections were documented in RV inspection report numbers 50-460/84-01 and 85-0 A follow-up inspection was performed to verify licensee compliance with extended construction delay commitments and NRC requirement The inspector sampled and reviewed the following Maintenance Project Instructions for significant changes since the previous NRC inspections noted abov . _ .

The majority of the surveillances involved maintenance and preservation requirement The licensee's computerized " Scheduled Maintenance System" (SMS) was reviewed and discussed with the licensee. Maintenance records, task cards, for Reactor Coolant Pumps were reviewed. The inspector discussed with the licensee IE information notice 85-56 regarding inadequate environmental control for components and systems in extended storage or lay u One of the examples noted in the information notice was the emergency diesel cooling water heat exchanger. The inspector determined that the WNP-1 diesel generator cooling water heat exchanger was not

included in the SMS. The SMS was developed by implementing vendor and A/E recommendations and by considering the relatively dry ' environment at the site. The SMS does not include all safety-related equipment. Items not included in the SMS (Ex. Diesel Generator Cooling Water Heat Exchanger) were considered part of a lot from which a sample of two items were selected and inspected each quarter. No record of inspection of the diesel generator cooling water heat exchanger was available prior to this inspection. The licensee was informed of an apparent need to list all safety related items not included in the SMS and a review of that list to conscientiously determine if any additional inspections or preservation requirement would be needed, taking into consideration the conditions noted in information notice 85-56. The licensee was informed that IE information notice 85-56 would be left open for further NRC follow-u .During the review of the SMS task cards, the inspector was informed that the original completed task cards were being maintained by the preventative maintsnance group and held there for two years. The cards are turned over to the QA vault after two years. The licensee stated that this allowed more convenient access to the cards and minimized QA vault personnel requirements. The inspector reminded the licensee that the tasks cards were important documentation of maintaining the plant in good condition for resumption of construction and subsequent operatio Accordingly, the QA record of the preventative maintenance activity should be maintained in the QA vault. The Project Quality Assurance Manager stated that the practice of maintaining the task cards in the preventative maintenance groups fire-proof cabinets under the control of the groups records keeper was adequate. Pending further inspection for determination of adequacy of the existing system of task card record maintenance, this was identified as inspector follow-up item 50-460/86-01-0 The inspector walked through various areas of the turbine building, the aux 111ary building, the containment, exterior laydown areas, warehouses 1, 3 and 5, Bechtel's QC records vault (bidg #3) and UEC QA vault (bidg

  1. 2). The noted areas were inspected for housekeeping, maintenance and access control. The inspector observed in warehouse 1/3 that valve 1-CCW-V-487 had a QC hold tag and was stored in the same pallet and bin with other QC accepted safety related valves such as 1-DHR-V-61 aad 1-FWA-V-1. The inspector was informed that tagging was site practice and segregation was not considered practical by Bechtel or the license Bechtel WNP 1/4 Construction QC Manual Section IV - Control of Nonconforming Items, paragraph 3.2.1 allows segregation or tagging of non-conforming material. However, ANSI N45.2.2-1972, Packaging, Shipping, Receiving, Storage and Handling of Items for Nuclear Power Plants, paragraph 5.3.2 requires nonconforming items to be identified and when practical segregated. The licensee noted that the current storage function of the Bechtel contract and the Bechtel procedure will be replaced by the Supply System in July of 1986. The licensee agreed to evaluate the need for segregating, when practical, nonconforming material. Since no significant safety related material issue or installation is currently being performed at WNP-1 nor is planned in the immediate future, this was identified as unresolved item 50-460/86-01-02 pending further review and inspection of the licensee's nonconforming material control requirement when it takes over this function from Bechtel in July, 198 .

Follow-up of IE Circulars, IE Information Notices and IE Bulletins The licensee's program for processing IE Circulars, IE Information Notices and IE Bulletins was reviewed and discussed with the licensee. WNP-1 Project Site Procedure (PSP) 3.5.6 Rev. 3, Processing of New Regulatory Information and~ Requirements Issued by NRC (during plant design and construction), provides the licensee's program for processing IE Circulars, IE Notices and Bulletins. The procedure provides requirements for distribution and review of the IE Circulars, Notices and Bulletins, determination of any action required and preparation of any required correc*' action. The licensee had a system for tracking and record keepint Circulars, Notices and Bulletins. A sampling of the licensee', -lons indicated compliance with PSP 3.5.6 based on the inspection, the tallowing IE Circulars and Notices were closed:

. IEB 79-21 - Temperature Effects on Level Measurement. The Bulletin was forwarded to power reactors with construction permits for information only. The Bulletin was close IEB 80-03 - Loss of Charcoal from Standard Type II, 2 Inch,' Tray Adsorber Cells. The Bulletin required licensees to inspect charcoal adsorber cells and report the potential for loss of charcoal incidental to handling, storage or use. The licensee response, dated March 19, 1980, indicated compliance with the requested sampling inspection and reported that the charcoal adsorber cells supplied to the site had screens that were welded to the casing and no separation, degradation or sagging of the screen would allow the charcoal to escape. The Bulletin was closed, IEB 80-08 - Examination of Containment Liner Penetration Weld. The licensee submitted its response on July 10, 1980. NUREG/CR 3053 provided a summary of the review for licensee responses to IEB 80-08 and provided closeout acticn. The NUREG closed IEB 80-08 for WNP- IEB 83-05 - ASME Nuclear Code Pumps and Spare Parts manufactured by the Hayward Tyler Pump Company (HTPC). The Bulletin required licensees to perform inspections or tests on HTPC pumps and spare parts. The licensee submitted its response to IEB 83-05 on August 10, 1985, and stated that no HTPC pumps or spare parts have been specified or purchased for WNP-1 safety-related application The Bulletin was close .

No violations or deviations were identifie . Unresolved Items Unresolved items are matters about which more information is required to determine whether they are acceptable or may involve violations or deviations. One new unresolved item identified during this inspection is discussed in paragraph . Exit Interview The inspection scope and findings were summarized on June 19, 1986, with those persons indicated in paragraph 1. The inspector described the areas inspected and discussed in detail the inspection findings. The licensee did not agree with the need for prompt QA control over the scheduled maintenance task cards identified in inspector follow-up item 86-01-01. No other dissenting comments were received from the licensee. The licensee did not identify as proprietary any of the material which was provided to or reviewed by the inspector during this inspection.
