APPENDIX A NOTICE OF VIOLATION Pacific Gas and Electric Company 77 Beale Street, Room 1451 San Francisco, California 94106 Docket No. 50-275 License No. DPR-80 As a result of the inspection conducted from April 7 through May 25, 1985 and in accordance with NRC Enforcement Policy,
CFR Part 2, Appendix C, the following violation was identified:
The Unit 1 Technical Specification Limiting Condition for Operation 3.0.4 specifies in part that "entry into an OPERATIONAL mode shall not be made unless the conditions for the Limiting Condition for Operation are met without reliance on provisions contained in the ACTION REQUIREMENTS...."
Unit 1 Technical Specification Iimiting Condition for Operation 3.8.1.1 specifies in part that three separate and independent diesel generators must be operable for Modes 1, 2, 3, and 4 of reactor operation.
Contrary to the above, on April 30, 1985, the licensee entered Mode 4 of reactor operation with only two operable diesel generators, and for a period of 10 days escalated, through Modes 3, 2, and 1 to 100/ reactor power (see body of report for details).,
This is a Severity I,evel IV violation,(Supplement 1).
Pursuant to the provj.sions of 10 CFR 2.201, Paci.fic Gas and Electric Company is hereby required to submit to this office within twenty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved:
(2)
corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
