Areas Inspected:
Routine, unannounced inspection by a regional based inspector of contractor's program for training and qualification of QC personnel; licensee site audits; and review of status of previous inspection findings.
The inspection involved 15 inspector hours on site by one NRC regional based inspector.
Resul ts:
One item of noncompliance was identified in the one area inspected.

The inspector also interviewed other licensee and contractor employees during the inspection.
2.
Plant Tour The inspector made a tour of the construction site to observe work activities in progress. The inspector examined work items for any obvious defects or noncompliance with regulatory requirements and for evidence of quality control of the work.
Specific activities observed by the inspector included installation of reinforcing steel for the containment foundation; and placing and compaction of structural backfill.
No items of noncompliance were identified.

During an inspection of this equipment by

Combustion Engineering (CE), they had committed to provide additional preventive maintenance instructions.
This information,
"Long Term Storage Procedure," Appendix 2, to Operating and Service Manual for Charging Pumps CH-P-1A, CH-P-18, CH-P-lC and CH-P-1D had been furnished with letter dated July 18, 1978, from CE to licensee.

The inspector examined Procedure CP-03-FR, Rev. 2, which classifies types of defects and establishes the require-ments for their repair.

The inspector reviewed the quality level (QL) designations and definitions with licensee representatives.
In some cases, QL designations include both safety related and non-safety related items.
Where such items are not clearly non-safety related they will be considered by NRC to be safety related pending clarification of their functions by the A-E.

The nonconformance had been documented on N&D Report No. 0306, which was closed on the basis of 7-day compressive strength tests of cylinders taken from the out-of-specification batches of concrete.
The inspector examined Concrete Cylinder Comp.'essive Strength Reports for the batches in question, which showed average 7-day strengths slightly above 3000 PSI and average 28-day strength of approximately 4000 PSI.
The specification requires 3000 PSI concrete.

Qualification and Training of QC personnel The inspector examined M-K Quality Control Manual, Procedure GEN-18 " Training and Qualification", dated April 15, 1976.
This procedure states that training of quality department personnel "..
encompasses the following:
Detailed review of Morrison-Knudsen's Quality Program.
a.

Review of Code and Standard requirements for the project, c.
Detailed reviev of project procedures and instructions.
d.
The function and implementation of the quality program.
Detailed review of Morrison-Knudsen's Welding Manual, e.
f.
Review of travelers, installation checklists, etc."
The inspector also examined the experience, qualifications and training records of QC personnel.
QC personnel had been qualified by education and previous experience in various civil construction disciplines and had been certified as level 2 inspectors.
The inspector then reviewed the training schedule and records and discussed the training program with M-K personnel.
The present program and schedule includes training in certain M-K construction procedures; and in M-K document control, material identification and material control.
However, the remainder of the training requirements outlined above are not scheduled and have not been included in the training program.
The inspector was informed by M-K QC supervisor personnel that on-the-job training and weekly training sessions were planned on new techniques, specification changes and special problems.
Failure to provide a complete training program as outlined in Procedure GEN-18 and to conduct such a program is considered to be in noncompliance with Criterion V of 10 CFR 50, Appendix B.

Site Audits The inspector examined GPU audits of M-K Nos. S-FR-1 78 05 and S-FR-78-08.
Positive action was taken by the auditor to assure corrective action including a letter dated July 21, 1978, providing

6.
Exit Inteview At the conclusion of the inspection on August 9, 1978, a meeting was held at the Forked River site with representatives of the licensee and contractor organization.
The inspector summarized the results of the inspection as described in this report.
