The NRC has received your letter dated January 26, 2024, and NRC Form 313A(AUS)
requesting to add Ewa Zabrocka, M.D., as an authorized user for Leksell Gamma Knife Icon. Unfortunately, the NRC Form 313A(AUS) lacks significant amounts of information in sections 3.a. and 3.b., and thus the NRC cannot make a determination of adequacy of training for Ewa Zabrocka, M.D. Therefore, the NRC has suspended the review of your letter dated January 26, 2024, and has closed mail control number 639017 without taking any action.
You can submit a separate license amendment request by sending a signed and dated letter containing a complete NRC Form 313A(AUS) and supporting documentation to R4Licensing@nrc.gov.
