C. Redding-2-1. Please confirm the following: "We will implement and maintain the operating, emergency, and security procedures in Appendix G to NUREG-1556, Vol. 1, Rev. 2, "Consolidated Guidance About Materials Licenses: Program Specific Guidance About Portable Gauge Licenses." Copies of these procedures will be provided to all gauge users and will be available provided as at each jobsite" (I've attached a copy of Appendix G for your convenience.)
2. Please confirm the following: "Leak tests will be performed at intervals approved by the NRC or an Agreement State and specified in the SSD registration certificate. Leak tests will be performed by an organization licensed by the NRC or an Agreement State to provide alternative leak testing services to other equipment licensees; or by using a leak test and/or sample collection kit supplied by procedures an organization licensed by the will be NRC or an Agreement State to provided as provide leak test kits and/or part of the sample analysis services to other application licensees and according to the kit and that supplier's instructions. Records of records of leak test results will be maintained."
3. Please note that you are using a dosimetry system (Direct-Ion Storage/lnstadose)
that is remotely processed. Please confirm that you understand the following:
a) The dosimetry system is designed for remote data extraction and digitally transferring data for evaluation to an accredited National Voluntary Laboratory Accreditation Program (NVLAP) processor; b) the dosimeters are provide~ and the dose of record is determined by a NVLAP accredited processor; and c) the licensee and NVLAP-accredited processor implement the necessary quality controls to ensure that the DIS dosimeter is calibrated at the appropriate intervals and/or replaced when circumstances warrant i. Please update the Region IV Office address and phone number on page 4 of the application. Additionally, please update the number to call for emergency/event involving radioactive material to Headquarter Operations Center (301) 816-5100.
