This is in reference to your request dated February 19, 1987 for amendment of

your byproduct material license.
In order to complete our review, we need the

following additional infonnation:
1.
Although your letter specified that radiation level surveys will be conducted during and after use of P-32, a minimum time-interval for these surveys'was not specified.- You should specify the frequency for
!
thesesurveys(daily, weekly,atintervalsnottoexceed30 calendar days). You may specify that these surveys will be conducted at intervals not to exceed 90 calendar days when licensed materials are in
storage and not in use.
2.
You should specify the name, address, and license number of the firin which will calibrate your Model GSM-5 survey meter. Also, state the minimum calibration frequency. The calibrations should be conducted at least once every 180 calendar days for instruments used in your laboratory.
3.
Although your letter states that areas where radioactive materials are located will be locked Attachment B indicates that P-32 will be stored
.in a freezer in a stairwell. You should specify how these materials will be secured to prevent unauthorized use or removal.
