Subject: NRC Inspection This refers to the in:;pection conducted by Mr. Paul Zurakowski of this office on June 22, 1979, of activities authorized by NRC License No. 50-15852-01 and to the discussion of cur findings held by Mr. Zucakowski with Mr. Mike Krueger at the conclusion of the inspection.
The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Comission's rules and regulations and the conditions of your license.
The inspection consisted of selective examinations of procedures and representative records, interviews with personnel and observations by the inspector.
During this inspection, it was found that certain of your activities appeared to be in nonconpliance with NRC requirements. The items and references to the per-tinent requirements are listed in the enclosure to this letter. These items have been categorized into levels as described in our correspondence to all NRC licensees dated December 31, 1974.
This notice is sent to you pursuant to the provisions of Section 2.201, of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations. Sec-tion 2.201 requires you to submit to this office, within twenty (20) days of your receipt of this notice, a written statement or explanation in reply includ-ing (1) corrective steps which have been taken by you, and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full ccmpliance will be achieved.
