I '" Summary: Insoection on December 17, 1979 (Report No. 15000046/79-32) As part of the inspeccion conducted during December 14-21, 1979 at NECO, Richland Washington, the inspection of trailer 440420 is detailed in this report.

State of Washington inspecror was on site.

One container was opened (paragraph 3).

Results: One container was found in an unclosed condition (Paragraph 3).

Another container had a punctured hole.

This centainer and 3 others were not labeled (Paragraphs 4 and 5).

All these items are in noncompliance with DOT regulations.

It appears that the Department of Transportation and the State of New York are the appropriate organizations to take enforcement actions.

DETAILS Persons Contacted Vern Apple, Site Manager, NECO Robert Phalen, Site Radiation Safety Officer, NECO Floyd Dickerson, Site Radiation Monitor, NECO Arnold Schreffler, State of Washington Department of Social and Health Services Background NRC inspection at the low level radioactive waste disposal site, operated by NECO at Richland, Washington, contiunes. This report, as part of the inspection conducted between December 14 and 21, 1979, details the inspection results for Trailer 44C,20.

Radiation survey instruments used were NRC 005340 (Xetex Model 303A) and NRC 000356 (Digi - Master).

The field count rate meters were NRC 004280 (TA PUC - LAB, s/n 10747) and NRC 004279 (TA PUG - 1AB, s/n 10743) connected to pancake GM detectors. All these portable instruments have a recalibration date of February 26, 1980.

Laboratory recounts of the wipe samples were analyzed with NRC 000383, NMC Gas Proportional Counter Model PC 55.

A representative of the State of Washington was present during this inspection. DOT representative was not onsite.

One container was opened.

As an independent measurement effort, this vehicle was surveyed for contamination after unloading.

The measurement was conducted with a direct survey using a pancake detector about 1 inch from the surface of interest.

The survey foufd no contamination on this vehicle.

Licensee - Shipper: Padiac Research Corporation Brooklyn, New York Shipment Inspected: December 17, 1979 Carrier: Tri - State Motor Transport Company Trailer: 440420 Freight .267314 1.

This trailer transported a load of 210 metal containers as Radio-active Materials N.O.S. Freight Bill 267314 is enclosed as Appendix A.

Each container was assigned DOT class white I, yellow II or III labels.

With the presence of DOT class III packages, the truck was properly placarded.

Radiation survey of the exterior surfaces of the truck showed maximum dose rates of 1.6 mR/hr at the surface of the trailer, and 0.01 mR/hr at 6 feet from the trailer, and in the cab.

Selective wipes showed no loose radioactive contamination.

Upon opening the rear doors of this trailer a 55 gallon metal container was observed with the circular ring disassembled and the closing bolt missing.

Enclosed Appendix B-1, B-2, and B-3 are reproduced photographs of the undisturbed load.

The container was identified as item 48733.

The missing bolt was observed lying on the floor of the truck be. . . -2-The cover was removed and placed on the bed.

The contents of the container was noted to be a masking tape sealed polyethylene bag with liquid scintillation vials and matertials with the appearance of compressed paper packing.

Enclosed Appendix B-4 is a reproduced photograph of the cover and the b-It.

B-5 is a photograph of the packaged contents. Wipes of the exterior of the containce showed no contamination.

Since this package was found in an unenclosed condition, this condition appeared to be in noncompliance with the DOT regulation uhich requires each closure to be properly closed and sealed (49 CFR 173. 393 (n) (6)). 4.

During the off-loading, container 66628 was observed with a punctured hole near the bottom of the container.

Dry packing material was seen thru this hole, but no packing material was observed on the truck bed.

Reproduced photographs, as Appendix C-1, C-2.and C-3, show this package. Wipes of the package showed no removable radioactive contamination.

This loss of container tightness appeared to be contrary to DOT requirements (49 CFR 173.393 (d) ). 5.

As evident from Appendix C, container 66628 was not labeled with DOT I, II or III in accordance with DOT regulations. Three other containers, identified as items $1854 56503 and 67175, were also not labeled.
