jnenardon Scone Amersham Corporation (Amersham) is authorized, in part, to perform emergency source retrieval operations at temporary jobsites anywhere in the Commonwealth of Massachusetts (Commonwealth) where the Commonwealth has jurisdiction under Commonwealth License No. 2012836-01 MA.

Qblervations and Findinas Amersham requested and was granted reciprocity by NRC Region 1 (RI) under 10 CFR 150,20 for a general license to perform an emergency source retrieval operation at the Tosco Refinery in Trainer, Pennsylvania on December 8,1997.
The RI staff contacted Amersham representatives for further information on the incident to determine the licensee involved and the circumstances behind the need for the source retriaval.
Amersham personnel were interviewed by the inspector. Pencil dosimeters were determined to be zorood prior to the source retrieval operation, alarming ratemeters, and pencil dosimeters were operational. Amersham personnel demonstrated that they had a good understanding of the company's emergency source retrieval procedures, c.
Conclusions No safety concerns were identified.

Observations and Findinas The inspector interviewed personnel, observed the source retrieval operation, and reviewed the documents necessary for Amersham to perform the operation under reciprocity and ship the licensed material back to their facility.
The Assistant Radiatier. Safety Officer (ARSO) for the client (MOS) described, to the inspector, the spearance of the.end of the drive cable as being sheared off with a few strands of frayed cable sticking out. The break occurred just behind the mating attachment of the drive cable.

The inspector performed confirmatory surveys using a Ludlum 14C, End Window Geiger Mueller survey meter (Serial Number 19614) and a Ludlum 19 MicroR survey meter Serial Number 15521). The Ludlum 14C radiation level measurements were in agreement with the licensee's while the Ludlum 19's radiation level measurements were higher. (i.e.,1.7 millirem / hour maximum versus 3.5 millirem / hour maximum) at the boundaries. The inspector interviewed MOS personnel and determined that no unauthorized personnel approached the boundaries. The above was confirmed by the inspector by interviewing TOSCO personnel.
The source retrieval company arrived on-site at 7:45 p.m. A conference was called to brief all personnel as to what to expect and the responsibilities of all parties. The area around the source was secured by MOS personnel and all unauthorized personnel were removed to shielded areas, the source was recovered at 10:45 p.m. and placed in an Amersham Tech Ops 680L source changer for transport back to Amersham's facilities. Amersham personnel stated that the drive cable would be examined to determine what may have caused the source disconnect. The inspector determined that Amersham personnel possessed the proper documents permitting them to perform the source retrieval operation and the shipment of the licensed material back to Amersham's facilities.
Estimates of radiation exposures to personnel, according to self reading pocket dosimeters were:

Conclusions No safety concerns were identified.
