.Introductibo

  • This form is satended as a tool for the observer to use in obsetving and evaluating an exercise. There are several different *eodules* correspond-ing to different observer assignments; an observer assigned to an E0C should have the "EOC Nodule ," an observer assigned to the Media Center should have the "Media Center Module" and so on. The module you have should correspond to your assignment.

You should bring the form along with you when you go to observe. The questions in it vill serve as a useful' guide concerning what to look for. You may vish to fill out some or all of the questions while on location.

Directly belou is a short explanation of what is in the form and how to fill it out; please read thi.s explanation as soon as possible.

Content of Tere and Instructions f or Use (1) Structure and Tornat Each nodule is composed of several sec-tions. Each section is about a partievlar part of the emer-gency activities there - f or exa=ple ,

  • equipment," "communi-cations,* or
  • dose assessment.* Each section has two parts.

The' first part is- a series of questions - requiring either yu, no, N/A, N/0 or, short answer responses. The second p.rt consists of a "Sum =.ary" and "Significant Observations."

e Questio6 isesponses Tou have four options when answering questions: Tes, No, N/A,.and N/0. 'N/A' stands 'f or "not applicable." Tor example, if the personnel observed did not have desineters, then the question. "Vere their dosime t e rs properly calibrated 7* vould be 'not applicable.' In some cases, a whole section may not be applicable. For example, if you vill be obse: *ing a local EOC, most likely the section (in your EOC codule) on "Dose Assessment

  • vill not be applicable, since dose assessment is rarely done at a local EOC. N/0 stands for *not observed;" meaning you do not know or cannot judge whether the activity was done

, properly. In this instance, no deficiency is implied. If something dould_ have been done . (or -

should have been there) but vasn't, then you should mark "No." An explanation is required whenever the "No* colue 1 = a rh ..:. In some instances, checking "No* does not mean that a probleu existed. This should be explained in the targin. Ordinarly, however, a *No", constitutes a problem or a

.- deficiency. This should be explained in detail in the "Significant Observations

(2) Sue:arv Eere you should vrite a brief essay sue =arizing your observations for that section, and give y'our overall evaluation. Put the facts recorded in all questions in perspective by adding information that vill help to explaJn the activities observed. You should indicate whether an adequate capability to protect the public vas demonstrated with regard to the subject matter of that section. g not simply answer the lead-in questions with a *Tes* or *No.' These ques- as are presented to give you, the observer, the context 1 a tihich the "Suassary* is to be written.

'6/ Release Time: 1: 55 pm 6-29-83 This is Robert J. Boulay, Director of the Massachusetts Civil Defense Agency. There has been an accident at the Pilg' rim Nuclear Power Plant site in Plymouth, Massachusetts. The event has been classified as a General Emergency.

Personnel from the Massachusetts Department of Public Health are at the plant site to r ovide continuing investigations of the severity of the accidenL. Their ongoing investigations include assessment of the status of plant safety systems, reviewing the consequences of existing and potential releases of radioactive material from the plant, review of local weather conditions and field monitoring of the levels of radioac-tivity in nearby towns.

The Department of Public Health staff has briefed the Governor on the status of the plant. Based on this' briefing, Governor Dukakis has ordered that protective actions be implemented for the area of the town of Plymouth bounded by Route 3 on the west, Summers Street to the south and to the Kingston Town Line. .

Persons currently in the Plymcuth area bounded by Route 3 on the weste Summers Street to the south and to the Kingston Town Line are advised to EVAC _UATE.

The Department of Public Health has determined that there is sur-ficient time to implement an orderly evacuation well in advance of the arrival of any signficant levels of radioactivity. Please make the following preparations before you evacuate:

, 1. Pack only essential items. Be sure, however, to bring enough I

seasonal clothing for up to three days, any prescription medici-nes and prescriptions you need, personal items and infant supplies.

2. Turn off lights and appliances, and secure your home or office as if you were going on vacation.

1 l 3. If you have livestock, leave them in best available shelters.

Pravide feed and water for three days.

4. Don't get on the road with a car in poor running condition or too little gas. Instead, try to carpool with a neighbor. If you need help, call your local Civil Defense office for assistance.

designated evacuation routes to the reception centers for your area.
