COLUMBIA GENERATING STATION, DOCKET NO. 50*397 INITIAL LICENSE WRITTEN EXAMINATION DOCUMENTATION

Dear Mr. Garchow:

Attached is the Initial License written examination documentation as delineated in ES-501, C.1 for the SRO and RO examinations that were given at Columbia Generating Station on October 2,2009.

Specifically, the following material is enclosed as an attachment:

Written exam performance analysis for questions missed by half or more of the applicants.

The Examination Security Agreement ES-201-3 will be faxed to you as soon as all post examination signatures are complete.

ILC-18 NRC Exam Deficiency Analysis An analysis was performed on the ILC-18 NRC Written Exam for each question missed by half or more of the applicants. An Action (ARlCR) has been assigned to address each identified weakness and entered into the plant Corrective Action Database. The analysis was performed in accordance with ES-403.D.3. This analysis is being forwarded to the NRC Chief Examiner in accordance with ES-501.C.1.a. All remediation activities added to the applicant's Post NRC Exam Checklist must be completed prior to assuming Licensed Duties.

RO Question 6 Weakness: Student knowledge deficiency of ABN-CR-EVAC bases. The program covers the required knowledge in L0000040.

Action: Remediated the specific knowledge deficiency during the exam review. A line item requiring the students to discuss ABN-CR-EVAC bases with an SRO or instructor will be added to the Post NRC Exam Checklist (due 10/15/09.) A line item to pass a quiz covering this topic will be added to the Post NRC Exam Checklist (due 10/15/09.)

RO Question 19 Weakness: The stem of the question requires clarification. The students did not understand what knowledge the question was testing. When asked directly which systems are fire protected, the students responded correctly.

Action: The stem of the question will be revised for clarification prior to being added to the Vision database (due 12/01109.) This clarification is an enhancement only and would not signal a need to re-grade the examination.

RO Question 24 Weakness: Student knowledge deficiency of communication requirements during refueling. The task to respond to a loss of communications during refueling is included on the Initial License Task Checklist.

Action: Remediated the specific knowledge deficiency during the exam review. A line item requiring the students to discuss communication requirements during refueling with an SRO or instructor will be added to the Post NRC Exam Checklist (due 10/15/09.) A line item to pass a quiz covering this topic will be added to the Post NRC Exam Checklist (due 10/15/09.)
