Title	Damaged Gauge
Event Description	The following information was provided by the Arizona Department of Health Services (Department) via email: The Department received notification from the licensee that an Instrotek 3500 Xplorer nuclear moisture/density gauge was run over by a water truck on a construction site. The source rod was bent but the source was still in the shielded position. The gauge contains 10 millicuries of Cs-137 and 40 millicuries of Am-241:Be. The Department has requested additional information and continues to investigate the event. Additional information will be provided as it is received in accordance with SA-300.
