| Title | Excessive Exposure |
| Event Description | The following information was provided by the Louisiana Department of Environmental Quality (LDEQ) via email:
LDEQ was notified by the Acuren Inspection Radiation Safety Officer (RSO) via the DEQ Radiation Hotline at approximately 1240 EST on February 6, 2023, concerning a possible industrial radiography camera excessive exposure. According to the RSO, at approximately 1025 at the Shell Norco Refinery, a possible excessive exposure occurred. Two radiographers did not crank the Ir-192 source completely back in the camera. The first radiographer had his hand close to where the source was located for approximately 10 seconds. The estimated whole-body exposure for both radiographers is 448 mR and the worst-case exposure to the hand for one of the radiographers is 96.86 R. The licensee is currently doing a reenactment to have a better idea of the potential exposures. The licensee is sending the radiographers for a blood count. The camera is a QSA Global 880 Delta (s/n D13733) with 41 curies of Ir-192 (s/n 54759). LA Event Report ID No.: LA 20230002 |
