Title	Gauge Shutter Failure
Event Description	On September 14, 2022, a generally licensed fixed gauge (Heuft Model 45US, SN: 2RG017177) containing 40 mCi of Am-241 (Source Model AMC.25, SN: 4479CW) failed to operate/function as designed. The on-off mechanism (e.g. shutter) of the device failed to return to the closed position. The licensee contacted the manufacturer and the device was removed from service. On September 16, 2022, the manufacturer arrived at the facility and repaired the device. Surveys and safety checks were performed by the manufacturer, and it was determined that the likelihood of anyone being exposed to excessive radiation levels is highly unlikely.
