| Title | Radiography Source Unable to Retract |
| Event Description | The following information was received from the state of Kansas via email:
Initial notification from State of KANSAS that we have a radiography licensee reporting that they have a source that is unable to retract. The licensee is a fixed radiography licensee and the source is exposed inside one of their main shops. The shop is not occupied, their [standard operating procedure] (SOP) is to have radiography work after hours when the work spaces are vacant. The area is secured by the radiographer/[assistant radiation safety officer] (ARSO) and his assistant. It appears the stand the guide tube was on fell [and] it is assumed the tube was damaged by the fallen stand. This is an initial report, more information will follow. The device contains a 24.8 Ci selenium-75 source. |
