| Event Description | The following is a summary of a phone call with Jacobi Geotechnical Engineering (licensee):
On December 3, 2021, while on a jobsite in St. Peters, MO, the licensee's Humbolt 5001EZ density gauge was run over. The source rod was broken, but remained secured in the case. The Humbolt gauge serial number is 3141 and contains 10 mCi of Cs-137 and 40 mCi Am-241/Be. The licensee requested assistance to secure and dispose the gauge. The gauge was secured and packaged in a transportation box. No overexposures were reported. |
