| Title | Inability to Retract Radiography Source to Shielded Position |
| Event Description | While the licensee was performing radiography work in Evansville, Indiana, a radiography camera source (SPEC-150; Ir-192; 46 Ci; Source S/N: ZE0316) became disconnected and did not return to the shielded position. The technician called the corporate radiation safety officer (RSO) at 1015 EDT on 8/17/2018, who then drove to the Indiana site. The RSO verified boundaries at the 2mR/hr level. The source was temporarily shielded while camera maintenance was performed. The source was returned to the camera, secured, and properly shielded within the camera. There was no indication of excessive dose. The RSO who retrieved the source received approximately 100 millirem (as indicated on their self-reading dosimeter). |
