Title	Agreement State Report - Loss of Ni-63 Gas Chromatograph Source
Event Description	The following information was received from the Commonwealth of Kentucky via email: On July 5, 2017, Murray State University [General License 401-111-20] reported the loss of an electron capture detector (ECD) contained in a Hewlett-Packard gas chromatograph model 6890. The ECD contained 15 mCi of Ni-63, S/N US00000603. The licensee stated they shipped the ECD on 1/14/16 by [common carrier] to Agilent Technologies, Inc. On 6/19/17 Agilent Technology verified with the licensee they had not received the device. The licensee believes the package containing the ECD was picked up with other items shipped that day by mistake. Kentucky Event Report ID No.: KY170006 THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
