The following is a synopsis of information received via E-mail:
On July 21, 2016, while in the process of performing an inspection at KRISS USA, Inc., during inventory performance, the Radiation Safety Officer discovered that a Hydrogen-3 sealed source (25 milliCuries, mb-microtec Model 400/1, SSDR #NY-1271-S-101-S, source lot #7036) was missing. The Radiation Safety Officer performed an investigation, which included interviewing users, and performed a visual inspection of the work area, of the path from the work area to the storage area, and of the storage area. A visual inspection of these areas was also conducted with the lights off to assist in the ability to locate the source. Upon failure to locate the source, based on information obtained through interviews with the user, the Radiation Safety Officer concluded that the source was likely lost in proximity of the work station. The most recent wipe test (July 6, 2016) of the work station was performed and analyzed by an appropriate service provider. All results found no detectable contamination. Sources that are "Less than
IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as
moisture density gauges or thickness
gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
