On April 13, 2016, the licensee reported to the Agency [Texas Department of State Health] that a radiography crew had failed to secure a
QSA 880 Delta industrial radiography camera (SN: D9092), which contained a 98.7 Curie
Iridium-192 source (SN: 30367G), inside their truck before leaving their facility to go to a temporary job site. The crew left the facility with the camera on the tailgate of their truck. A member of the public saw the camera in the street at an intersection approximately 100 yards from the facility. He knew what the camera was and that the licensee had a facility there. He moved the camera out of the street and into the ditch and called the licensee. The licensee responded immediately. The licensee inspected the camera and found no damage. They surveyed and confirmed the source was still in the fully shielded position. The licensee's radiation safety officer reported initial estimate is that the camera was out of their possession approximately 5 minutes and no one received any exposure above regulatory limits. The licensee will complete its investigation of the incident and will confirm or correct this initial information. Since the licensee has not yet submitted a dose assessment for the member of the public, we are making this report. More information will be provided as it is obtained in accordance with SA-300.
