| Title | Texas Agreement State Report - Broken Source Operating Cable |
| Event Description | The following information was received from the state of Texas via email:
On October 19, 2015, the Agency [Texas Department of State Health Services] was notified by the licensee that the operating cable on a 150 milliCurie Cesium - 137 source used in a Berthold model 21357 nuclear gauge broke during routine operation of the source. The licensee stated the source is stored in the fully shielded position. The licensee stated the source does not create an exposure risk to its personnel or any member of the general public. The licensee has contacted the manufacturer for assistance in repairing the gauge. Additional information will be provided as it is received in accordance with SA-300. Texas Incident #: I-9350 |
