Title	Medical Misadministration During Iodine 125 Seed Implantation
Event Description	During a medical procedure to implant 70 Iodine-125 seeds (0.4 mCi each) into the prostate, 20 were implanted into the bladder. The 20 implanted into the bladder were recovered during the procedure. There was no effect to the patient. The patient and prescribing physician have been notified. The licensee is working on corrective actions to prevent a recurrence. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
