Title	Patient Received Wrong Formulation of Technetium Radiopharmaceutical
Event Description	A patient who was to be given a myocardial stress profusion exam was incorrectly given a 160 mCi dose of technetium pertechnetate [instead of prescribed Tc-99m sestamibi]. The cause of the incident was the medical technologist deviated from established procedures. There is no expected adverse impact on the patient as a result. The patient and physician have been informed. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
