Title	Agreement State Report - Underdose from a Y-90 Procedure
Event Description	The following report was received via email: [The Florida Bureau of Radiation Control] received a call from [the licensee's RSO] regarding an underdose incident on a patient. During the procedure, the technician noticed bubbles in the administration line, and stopped the procedure. It was estimated that the patient only received 4 mCi out of the 16 mCi dose prescribed. [The licensee's RSO] will be sending a full report of the incident to this office. [The Florida Bureau of Radiation Control] will assign an inspector to investigate. Florida Incident Number: FL 141-046 A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
