Title	Wrong Form of Radioactive Isotope Administered for Cardiac Stress Test
Event Description	A patient received a dose of MDP (25 mCi Tc-99m) which is used for bone scans vice cardiolite (31 mCi Tc-99m) for a cardiac stress test. The wrong dose was administered due to technician error. Both the prescribing physician and patient have been informed. No adverse health effects are anticipated. The licensee has corrective actions under review. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient. * * RETRACTION FROM JENNIFER BAILEY TO DONG PARK AT 1012 EDT ON 5/21/2014 * * * Retraction of Event # 50121 [is requested] for Beckley Appalachian Regional Healthcare, License # 47-17725-02. The event reported did not exceed 5 rem as specified in section 35.3045 (a) (1)." Notified the R1DO (Gray) and FSME Events Resource via email.
