Title	Agreement State Report - Possible Disconnect of Industrial Radiography Equipment Source
Event Description	The following information was provided by the State of Kansas via email: Industrial radiography equipment failed to function as designed which resulted in a possible source disconnect. The radiography equipment was located at a work site in Winfield, Kansas. The radiography equipment contained a 56 Curie Ir-192 source.
