| Event Description | The following information was received via email from the Illinois Emergency Management Agency:
On December 19, 2012 this general license registrant called to advise that a shutter on one of their process gauges had failed. The registrant had discovered the failure as a result of conducting their routine 6-month safety checks of the device. A makeshift shield was placed over the beam opening following the manufacturer's instructions, pending repair. Arrangements were made to have a field engineer from the company come to the site the next day to replace the shutter. No exposures to operating personnel were reported due to the circumstances of the gauge installation and low activity of the source involved.
