Title	Agreement State Report - Medical Under Dosage
Event Description	The licensee notified the State of Oregon of a misadministration of "Sirspheres" during the treatment for liver cancer. The dosage of radioisotope Y-90 administered was 34.2% less than that prescribed. The patient was informed of the under dosage. The patient may be rescheduled to receive an additional treatment. The State is investigating. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
