Title	Agreement State Report - Broken Gauge
Event Description	On September 17, 2008 the licensee reported that a[n] Ohmart/Vega (OV) model SH-F1 has a sheared bolt on the shutter handle which prevents it from closing. This device (S/N 1297GK) contains 20mCi of Cs-137 in a model OV A-2100 sealed source capsule. The manufacturer has been notified and will see to the proper repair or replacement of the gauge. No public exposures are likely to approach regulatory limits and the vessel has been posted to prevent entry.
