At about 11:10 EDT on 10/4/07 during a routine
gauge inventory identified that a Thermo Measuretech (formerly
Texas Nuclear) model TN-5192 fluid density
gauge could not be accounted for. The
gauge serial numbers are D111 and MB2856. The
gauge has a 200 millicurie
Cs-137 source.
There is no sign of theft but the gauge has not been found at its expected storage location or any of the well sites in the area. According to records it was last used in April 2007 and last inventoried in August 2007. Schlumberger will expand the search to include all facilities in the country.
THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL
Sources that are "Less than
IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as
moisture density gauges or thickness
gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
